Main duties of the job

  • Manage a varied caseload, conducting Care Act Needs Assessments using strength-based, person-centred approaches.
  • Work closely with colleagues from health, education, and partner agencies to support young people and their families
  • Always promote independence and the least restrictive practices.

About you

  • Minimum of two years' post-qualification experience.
  • DipSW or equivalent qualification.
  • Social Work England registration

Key duties include completing Care Act assessments using strengthbased, personcentred practice and holding case responsibility for people in the Transitions service. The role involves promoting independence, reviewing plans, managing risk, and ensuring leastrestrictive approaches. You will work closely with social care practitioners, the transitions coordinator, and partner agencies to deliver coordinated support. Responsibilities include accurate recording, interpreting statutory duties, providing guidance to the team, supporting professional development, and maintaining strong multiagency relationships. The role also requires identifying safeguarding concerns, understanding local services, and advocating for individuals to ensure appropriate support and highquality outcomes.

  • Qualified Social Worker with evidence of post qualifying learning and development and SWE registration.
  • Up-to-date knowledge of relevant legislation and guidance in relation to working with, and the safeguarding of, children and vulnerable adults.

Experience

  • Evidence of giving informal supervision.
  • Demonstrable commitment to practice development through self-reflection.
  • Well-developed interpersonal skills and ability to effectively communicate with people in a variety of ways and levels.
  • Good IT skills and a proven ability to keep accurate records.
  • A self-starter, well organised person who is passionate about delivery of high-quality customer-centric services.
  • Ability to work with others in a developmental way.
  • Ability to problem solve in complex and unpredictable situations.
  • Must be able to write clear, complex and accurate reports.
  • Fully fluent in English.

Desirable
