Learning Disabilities & Autism Service Supporting Independence and Well-Being

As an Experienced Social Worker, you will manage a varied caseload, conducting Care Act Needs Assessments using strength-based, person-centred approaches. Youll work closely with colleagues from health, education and partner agencies to support young people and their families, always promoting independence and the least restrictive practices.
Main duties of the job
The Social Worker Level 3 in the Transitions Service supports young people aged 1725 as they move from childrens to adult social care. The role involves completing strengthsbased, personcentred Care Act assessments to identify needs and develop effective support plans. You will act as the named worker for allocated young people, promoting independence and ensuring eligible needs are met. Working closely with childrens social care, you will help build early relationships and ensure a smooth handover when a young person turns 18.
A key part of the role includes regularly reviewing support plans, recording outcomes, and adjusting plans so they remain appropriate and least restrictive. You will provide guidance and informal supervision to colleagues, helping maintain good practice within the wider team. Collaborative working is essential, involving coordination with partners, professionals and other agencies to deliver joinedup, highquality support.
You will assess and manage risk, follow safeguarding procedures, and act promptly where concerns arise. Accurate and timely recording of all assessments, decisions and contact is required. The role also involves applying relevant legislation, policies and practice standards, maintaining uptodate knowledge of local services, and signposting young people effectively. Professional representation of the council, as well as active engagement in supervision and ongoing development, form key expectations of the role.

Job Purpose
The Transitions service works with people aged 17-25 to support people to move into adult services.
This will include people are part of the transitional safeguarding cohort who require additional
support to move into adult services.
Young people will remain with childrens social care until they turn 18. The people open to the
transitions team will have a named worker.
The transitions service staff will work alongside the childrens worker to build a relationship with the
young person and ensure that there is smooth handover between services.
The transitions service can start to assess people under adult social care legislation and plan for
what support is needed so that when they turn 18 the appropriate support will be in place
When people turn 25, they will move into the appropriate adult social care team should they still
require ongoing support. This will require close liaison between the transitions service and adult
social care.

Provide support and guidance to the wider team on case management, promoting least restrictive
approaches.
Professional supervision, training and effective relationship development with Level 2 social workers
