Wiltshire Reablement - Empowering Independence

Our Reablement Support Workers provide vital assistance to individuals in their homes post-hospital discharge. They play a pivotal role in helping customers achieve their goals, working closely with dedicated occupational therapists to co-develop personalised reablement plans.
We are seeking individuals with a genuine passion for helping others who are available on a rota basis between 7am to 10pm 365 days a year.
Main duties of the job
Reablement is a short-term, therapist led service aiming to prevent, reduce and delay the need for
ongoing services. Reablement supports people to be as independent as possible and includes the
provision of information and advice, analysis of needs, provision of equipment and adaptations and
targeted assistance from a team of support workers.
The reablement team seek to support people to make use of community and personal assets to
achieve their goals.
The overall responsibilities of the reablement team are:
To work with customers and carers/families to identify goals and design creative reablement
support plans to achieve them.
To connect customers with local resources and facilities.
To operate within legal frameworks and defined budgets for social care.
The team will work closely with colleagues from Health, private providers, the voluntary sector
and others, to support the occupational needs of customers.

Job responsibilities
Under the guidance and supervision of the Senior Reablement Worker (and from time to time other
members of the reablement service management team), your role will be to assist customers to achieve the goals and objectives set-out in their person-centred
reablement support plan.
This will include: providing customers with personal care and facilitating
customers with daily living activities.
Specific duties and responsibilities include:
Work with customers to help them regain their independence with daily living skills, including
washing, dressing, meal preparation, and personal care
Safeguard the health, safety and wellbeing of customers as well as colleagues
Take part in staff development programmes, assuming responsibility for own professional
development i.e. undertake all mandatory training relevant to the service and part of the training
pathway for the role e.g. Care Certificate and specialist reablement training
Assist customers with the use of mobility aids and assistive equipment where required
Deliver reablement support in a range of settings. Primarily customers own homes but, if
required, also day centres, day hospitals and in the community
Assist customers with moving and positioning, adhering to health & safety requirements as well
as organisational policy and procedures
Contribute to daily communications, accurate record keeping, recording actions, interventions
and incidents in customers records
Participate in regular team meetings and customer review meetings
Ensure compliance with the Data Protection Act and maintain customer confidentiality and
security of information at all times
Communicate effectively with the immediate team and other health and social care
professionals as appropriate
Participate in own supervisions, appraisals and objective setting
Assist the team with general duties if required e.g. office filing, answering telephone calls etc
Work as part of the team, offering a flexible approach to staff rostering
Abide by the principles and practice set out in the staff handbook
