Main duties of the job

  • Handling referrals and gathering essential information to support placement decisions.
  • Maintaining and updating databases and documentation.
  • Supporting the team with scheduling, communications, and training logistics.
  • Providing clear and sensitive guidance to carers and colleagues.
  • Data collection and organisation
  • Assisting with quality assurance and compliance monitoring.
  • Marketing and organisations of our resources, such as socials, to share information about our service
  • A clean driving licence and willingness to travel within Wiltshire is essential. While our base is in Trowbridge, we offer flexible working arrangements to support work-life balance.

Job responsibilities

Promoting Shared Lives Arrangements

  • The whole team is responsible for continuously promoting the work of the service, exploring carer recruitment opportunities and ensuring as many effective placements are arranged as possible.
  • Assist in the provision of general information about the Shared Lives Service; for example, take enquiries, explain how to make a referral, send out referral forms etc.
  • Take referrals from care managers, ensuring that sufficient information is gathered about the prospective customer to help determine the persons suitability for Shared Lives.
  • Using the information gathered above, assist the Shared Lives Officer within the scheme in the matching process of prospective customers with Shared Lives carers.
  • Assist in the arrangement of introductory meetings between people interested in receiving a Shared Lives service and Shared Lives carers; for example, help co-ordinate visit times, duration, etc.
  • Arrange for, and complete, Disclosure and Barring Service (DBS) and local authority checks. Take responsibility for ensuring that the references set out in the services policies and procedures are obtained.
  • Send reference request letters for prospective Shared Lives carers.
  • Attend preparation and training meetings and events for prospective Shared Lives carers.
  • Maintain accurate and up-to-date databases, ensuring compliance with data protection and safeguarding policies.
  • Support the coordination of carer reviews, panel meetings, and quality assurance processes.
  • Help prepare reports, presentations, and promotional materials for internal and external stakeholders.
Supporting Our Placements

  • Provide support and guidance, explaining and promoting good practice to Shared Lives Carers. Analyse behaviours, interpret situations and advise on interventions to achieve outcomes. And, where necessary, arrange for specialist advice or training to be provided by other professionals. More complex issues should be raised with the Shared Lives Officer.
  • Ensure all contacts, reviews and documentation amendments are well-recorded.
  • Provide information to colleagues in other teams as necessary to assist them to set up payments to the Shared Lives carer.
  • Liaise, as necessary, with colleagues throughout the duration of placement to ensure correct payments to Shared Lives carers are maintained. Act as a friendly and approachable point of contact for Shared Lives customers and carers.
  • Support customers with queries, signposting them to appropriate services or team members.
  • Assist in gathering feedback from customers and carers to help improve the service.
  • Help coordinate social events, training sessions, and community engagement activities for customers and carers.
  • Promote a person-centred approach, ensuring that the voices of customers are heard and respected.
General

  • Undertake office administration procedures as required (e.g. make arrangements for overseeing the collection of monies owed to the Shared Lives service and/or liaising with workers from other teams, administer the teams petty cash float).
  • Support in the set up of finance and payments for our placements with social workers and the finance team including, but not limited to; creating invoices, sending out and receiving invoices, creating funding agreements and chasing CPLI lines on LAS as well as leasing with the finance team on outstanding payments.
  • Assist in the planning events for Shared Lives carers; for example help to arrange training venues, send out dates of meetings to attendees and participate in discussions with carers at training events.
  • Together with Shared Lives team colleagues assist in the creation and delivery of advertisements and other information designed to attract new Shared Lives carers to the service. Explore new ways of recruiting carers, including, but not limited to, social media and developing an online presence.
  • Carry out all duties and ensure that Shared Lives carers perform their role in accordance with the requirements of the Care Quality Commission and relevant legislation including: The Care Act 2014, The Mental Capacity Act 2005, The Data Protection Act 1998.
  • Contribute to reviews and quality audits of the Shared Lives service, including assisting in the production of business plans and reviewing policies and procedures.
  • Stay up to date with practice developments, national guidance, new legislation and procedures, including the Care Act 2014 and the Mental Capacity Act 2005. Implement changes where required based on new information / guidance.
  • Follow both council and service policies and procedures, including, not lot limited to maintaining confidentiality whilst promoting a safe and effective service. Take appropriate action where any concern or risk to adults or children is identified.
  • Creating and sending out letters to our placements/carers
  • Creation and sharing of our quarterly newsletter
  • Editing and sharing our vacancy lists to share with stakeholders every month
  • Ensuring our presentation are up to date and accurate and presenting to stakeholder teams
