Job summary
Reablement - Empowering Independence
Were looking for a compassionate and skilled professional to join our team, supporting people in their journey to improved mobility, independence, and wellbeing.
In this role, youll work with a range of conditions, from neurological conditions like stroke and Parkinsons to musculoskeletal issues such as arthritis and fractures. Youll also support patients recovering after hospital discharge, working closely with our Reablement team to encourage mobility and maximise their potential.
Youll assess, treat, and manage patients, tailoring treatment plans to their individual needs and adjusting them as they progress. Your ability to communicate complex clinical information in a clear, supportive way will be essential, as will your ability to work with patients facing communication or cognitive challenges. Youll also play a key role in safeguarding, ensuring the highest standards of care and professional practice.

Were looking for someone with a degree in physiotherapy, HCPC registration, and experience working with diverse and sometimes challenging client groups. Youll need excellent interpersonal skills, a proactive mindset, and the ability to travel to patients homes and other care settings.
