Would you like to workin the community as part of our well-trained team of Occupational Therapists and support workers,creating person centred personal plans helping people set and achieve their goals following an illness? Then this role is for you.

Due to expansion, we have opportunities available to visit people in their own homes to assess their social care needs across a range of locations. Norfolk First Support delivers services to adults across Norfolk in their own homes for up to 6 weeks, assisting them to regain skills they may have lost following an illness or hospital admission, enabling them to regain their independence as much as possible while also reviewing any ongoing support needed.

Main duties of the job

In this role, you will be part of a multidisciplinary assessment team, working alongside other professionals in Social Caresupporting people who may have beendischarged from hospital or where they have a change of circumstances and require support at home.

You will assess and review their daily living needs, developing a personalised plan to assist them to regain the skills and confidence to remain independent in their home, and if ongoing services are required complete a Care Act Assessment. All training will be provided to ensure you are competent to deliver this critical role.

You will need to have experience of working with people and be able to communicate clearly and appropriately with members of the public, whilst exercising judgement and using your initiative to plan and prioritise your work. You will also need knowledge of current policy, practice and legislation within Health and Social Care.

Principal Accountabilities

To provide rapid response assessments, support and review persons/carers needs and make consequent arrangements within the agreed indicators. This includes working with people to carry out Reablement assessments/reviews within the community and complete support plans/Care Act assessments as appropriate with aim of maximising independence.

To work as part of the multi-disciplinary team liaising with other professional and services as necessary to provide the best possible care to people and their carers, this includes participating in regular MDT meetings.

To provide planned/unplanned support according to the persons need or following the person-centred plan/associated risk assessments (if applicable) to ensure people are provided with support that meets their individual needs.

Liaise with the Operational Manager, Service Delivery Lead and Registered Service Manager on issues relating to the delivery of Reablement Support such as deployment of staff and levels of care required.

To assess levels of support required post reablement and ensure smooth transition to alternative provider agency.

To bring to the notice of a qualified worker (Social Worker, Occupational Therapist etc) changes in a persons needs or circumstances when the nature of presenting difficulties fall outside agreed scope of this role.

To receive new referrals for people identified as requiring the assessment and reablement service, liaise with professionals regarding the individuals personal plan, and visit new people to introduce and provide information on the service within service timescales.

To operate in accordance with NCC and departmental practice and guidance, including the maintenance of electronic social care records within LiquidLogic, and relevant departmental and corporate administrative procedures.

To act as information point for other agencies regarding the persons care and support needs. To also signpost the person/family to alternative support within the community.

To promote Safeguarding awareness with service user and multi-disciplinary colleagues. Keep updated on current Safeguarding guidance/practice and be vigilant in all activities. To understand and execute the actions to take if concerns are raised.
