We represent a Local Authority dedicated to enhancing the well-being of our community and residents. We are currently seeking a dynamic and skilled Senior Occupational Therapist to join our Discharge to Assess Hospital Team. As a Senior Occupational Therapist at Reading Borough Council, you will become an integral part of a welcoming and collaborative team. We are in search of an individual with excellent communication skills, capable of establishing and nurturing relationships with colleagues, service users, and healthcare providers.The Hospital Discharge to Assess Team advocates a "home first" philosophy, employing a strength-based approach with a focus on reablement. In this role, you will be involved in identifying reablement goals, evaluating manual handling, and tailoring care packages for individuals as they transition from the hospital setting.This position provides chances for enhancing your expertise in Adult Social Care through engaging in high-quality, client-centred occupational therapy assessments and interventions. These activities occur in the homes of service users, aiming to assist them in cultivating, regaining, or sustaining meaningful occupations. All of this takes place within collaborative and supportive teams.
Main duties of the job
About the Role:You will hold a varied caseload and contribute to the team performance and positive outcomes for our residents. You will assist individuals in staying in their homes and optimizing their independence. Utilizing a strengths-based methodology and your distinct OT skillset, you will help individuals discover solutions, enhance well-being, and engage in meaningful activities through various interventions. This includes recommending equipment, suggesting adaptations, addressing mobility and handling issues, facilitating goal planning, and collaborating closely with care providers, Housing Services, and healthcare colleagues.You will work within the legal framework provided by the statute, guidance, policies, and procedures.Within your duties of Senior Occupational Therapist you will be required for provided supervision to the Occupational Therapists and Social Care Coordinators within the team.You will need to have:
- A degree in Occupational Therapy
- Registered with HCPC
- Community experience is helpful but not essential
