Job summary

An exciting opportunity has opened for a Specialist SCPHN School Nurse who is passionate about making a difference to children and families lives across Southend to join our School Nursing team.

You will work as an autonomous practitioner to develop provisions of a proactive and responsive delivery of the 5-19 Healthy Child Programme service to support the health and emotional wellbeing needs of Children and Young People who are Educated Other Than At School and are not attending mainstream school. This cohort will include young people attending Pupil Referral Units, Alternative Education Provision, and home tutoring.

You will proactively Support children missing from education to engage with an educational provision through referral and escalation to the Early Help/Child Missing from Education (CME) team. You will be the named school nurse for Victory Park School, providing drop-in clinics and targeted health intervention offers to students in Year 9 to support preparation for transition.

You will work closely with the Specialist School Nurse for Electively Home Educated Children and will share attendance in rotation at the EHCP panel meetings. The AP/EOTAS post holder will act as the SEND champion for the School Nursing Service.

Main duties of the job

You will provide a proactive and responsive service, working flexibly to meet the needs of children and young people in this client group utilising a variety of communicative tools such as face to face appointments, telephone contact, video calls and Chat Health text service.

You will develop and provide an engagement route for health assessments and discussions by setting up and maintaining nurse led drop-in clinics for young people and parents/carers for Young People who are Educated Other Than At School. This will include the development of evidence-based health promotion and educational sessions for delivery to children and young people to support increased knowledge and understanding of effectively managing health needs.

Arranging meetings between parents and education to support the development of health needs care plans and effective implementation. You will attend the annual SENDCO meeting, to support manging the expectations from the schools for this cohort of children attending Alternative Provisions.

You will link with the inclusion team and review any health needs of young People being discussed at the inclusion panel. You will proactively Support children missing from education to engage with an educational provision through referral and escalation to the Early Help/Child Missing from Education (CME) team.

In Southend our school nursing and health visiting teams operate within an integrated 0-19 Childrens Public Health service model and incorporate a skill mix team approach to support aspects of service delivery.

This includes adhering to the local SET and national safeguarding policies and procedures, accessing mandatory safeguarding supervision, liaising with the locality safeguarding team and social care, writing safeguarding reports and attending Child Protection conference, core groups and Child in Need meetings.

Completion of safeguarding core health assessments to support in the identification of health needs and inform a personalised plan of care/interventions in collaboration with the client and their parent/carer.

To establish and maintain relationships with voluntary agencies to support the understanding of health needs, increase accessibility to services and address inequalities.

Support effective service responses and pathways where there are multi-agency service interfaces required.

To maintain an excellent standard of records for the service.

It is recognised that children and young people within this client group often present with complex and/or enduring needs which require a multidisciplinary and flexible approach in the aim of recognising and addressing barriers to services and reducing health inequalities. Due to these factors, children and young people in this client group may be at increased vulnerability.

To promote and upskill the Southend Children, Young People & Family Public Health service and other agencies regarding this client group.
