The Social Care Practitioner role is within the Integrated care team (LAP) which consists of Community Matrons, Mental Health Practitioners, Social Workers and Occupational Therapists.
The LAP team involves supporting people with multidisciplinary support to look at the needs of the resident in a holistic way helping to prevent longer term challenges and admissions to hospital.
Main duties of the job
- Assessments and interventions for residents with physical disabilities, not excluding cognitive decline
- Working with other professionals to support identified outcomes
- Holding and managing a case load
