We have an exciting opportunity for a Social Care Practitioner to join us!
This is a full-time, permanent role with hybrid working, and a salary of £26,895 - £30,132 per annum.
Main duties of the job
As an Adult Social Care Practitioner, youll play a vital role in supporting residentswhether in their own homes, hospitals, or care settingsby assessing their needs and coordinating tailored support that promotes independence and wellbeing.
The Social Care Practitioner role is within the Integrated care team (LAP) which consists of Community Matrons, Mental Health Practitioners, Social Workers and Occupational Therapists.
The LAP team involves supporting people with multidisciplinary support to look at the needsof the resident in a holistic way helping to prevent longer term challenges and admissions to hospital.

This role involves assessing and supporting adults in line with the Care Act 2014. Youll manage a caseload, work collaboratively with individuals, families, and professionals, and coordinate services that promote independence and wellbeing. Youll also contribute to team performance, maintain accurate records, and uphold the councils values.
