This is a full-time, permanent role based in the Town Hall in Maidenhead with some hybrid working, and a salary of £24,895 - £28,132 per annum. This is a great opportunity for anyone wanting to work in Adult Social Care.

Main duties of the job

The Social Care Practitioner role:

As an Adult Social Care Practitioner, youll play a vital role in supporting residentswhether in their own homes, hospitals, or care settingsby assessing their needs and coordinating tailored support that promotes independence and wellbeing.

Your role will involve:

This role involves assessing and supporting adults with care needs in line with the Care Act 2014. Youll manage a caseload, work collaboratively with individuals, families, and professionals, and coordinate services that promote independence and wellbeing. Key responsibilities include safeguarding, risk management, support planning, and ensuring compliance with legislation, policies, and budgetary frameworks. Youll also contribute to team performance, maintain accurate records, and uphold the councils values and governance standards.
