Main duties of the job
As a Social Worker within these teams you will be helping to support adults to live independently and safely by assessing needs, coordinating care, and safeguarding wellbeing. Work closely with individuals, families, and partner agencies to promote dignity, choice, and empowerment.
You will be supported with your caseload by a senior Social Worker and a Team Manager.

Your efforts will help ensure that individuals transition smoothly from hospital care back into the community, with the right support in place tomaintain their wellbeing and independence.
Join theCommunity Teamand take on a rewarding role where youll handle more complex cases and contribute to case reviews. Your work will help ensure that residents are supported in living their lives to the fullest. This is a chance to make a real difference while developing your skills in a dynamic, collaborative environment.
This is a full-time, permanent role with hybrid working, and a salary of ASYE £36,071 per annumSocial Worker £38,192 - £42,012 per annum.
Your role will involve:
- Support & Advocacy: They help adults who may be vulnerable due to age, disability, illness, or mental health challenges to live independently and with dignity.
- Assessment & Planning: They assess individuals needs and develop care plans tailored to improve wellbeing and safety.
- Safeguarding: They protect adults from abuse, neglect, or exploitation, working closely with other agencies when risks are identified.
- Coordination: They liaise with families, carers, health professionals, and community services to ensure holistic support.
- Empowerment: They promote choice and control, helping people understand their rights and access services.
