We are recruiting a knowledgeable and compassionate Service Team Manager to lead our Hospital Discharge Team at the Royal Borough of Windsor & Maidenhead. Thisimportant role is central to delivering our Social Care Futures vision and ensuring that hospital discharges are safe, timely, and person-centred.

As Service Team Manager for the Hospital Discharge Team, you will lead a multidisciplinary team working across acute settings to ensure safe and person-centred discharges. You will:
Provide visible, supportive leadership to staff, promoting resilience and reflective practice.
Coordinate effectively across health and social care partners to advocate for individuals rights and needs.
Ensure compliance with statutory duties including the Care Act, Mental Capacity Act, Continuing Health Care, and safeguarding.
Champion strengths-based and person-centred practice in a fast-paced, multidisciplinary environment.
Contribute to RBWMs strategic priorities: building inclusive, healthy communities, promoting prevention and choice, and delivering compassionate, high-quality services.

- Be a professionally qualified Social Worker holding either CQSW, DiPSW, Degree in Social Work or equivalent.
- Registered with Social Work England
- PEPS 2 qualified and experienced practice educator (desirable)
- Qualified as a Best Interest Assessor (desirable)
