The Social Care Practitioner role is within the Front Door Team. The Team is the initial point of contact for people within the borough who are making contact with social services for the first time. The team provide advice, signposting to alternative services, onward referrals, occupational therapy equipment as well as completing statutory assessments in line with the Care Act 2014.
Main duties of the job
- Assessments and interventions with residents
- Signposting to other services and advice to residents contacting the team
- Holding and managing a case load
