We have exciting opportunities for Senior Social Workers to join the Hospital Discharge Team!

These are full-time, permanent roles, with a salary of £45,831 - £50,414 per annum plus an annual retention bonus of £3,000 subject to eligibility.

Main duties of the job

The Royal Borough of Windsor and Maidenhead (RBWM) is looking for Senior Social Workers to join the Hospital Team. This is an exciting opportunity to make a meaningful difference in the lives of local residents while contributing to a high-performing and supportive workplace.

The Role:

As a Senior Social Worker, you will deliver statutory services to residents with eligible care needs by conducting outcome-focused assessments, creating tailored care plans, and ensuring their effective implementation. Alongside managing a caseload of complex assessments, you will provide supervision and support to team members, fostering their professional development and ensuring high-quality service delivery.

You will work flexibly with the management team, lead on key projects, and chair meetings such as risk assessments and best interest decisions. With a focus on safeguarding vulnerable adults, achieving performance targets, and operating within budgetary constraints, you will play a vital role in driving continuous improvement and making a meaningful difference in the lives of our residents.

About the team:

The Hospital Discharge Team works collaboratively within a multi-disciplinary environment to provide support for people who may require adult social care services to support their safe and timely discharge from hospital. The team provides high quality strength based assessments and support plans underpinned by Section 9 of the Care Act 2014, for individuals in consultation with the person, family and promote their choice and independence.

The social work team engages patients and family members in the transition from hospital to home, with the goal of reducing adverse events and preventable readmissions.

To execute these tasks, the team undertakes the full range of social work tasks as required by the needs of each case and the policies of the department in order to provide the best possible service to service users.

  • Proven and exceptional ability to challenge service users and carers where the thinking will not result in a Support Plan that is fit for purpose.
  • Skills in supervisory/appraisal and adult learning processes.
  • Ability to participate in a wide range of meetings, including chairing meetings on a regular basis.
  • Knowledge of the local social care market, along with ability to source further information.
  • IT Skills competent in use of Microsoft Outlook and Word.

Desirable

  • Ability to learn to utilise the electronic workload management and information system.
  • Good negotiating and time management and delegation skills, including ability to manage own workload and that of others.
  • Good communication skills, both oral and written to include report writing.
  • Ability to problem solve at a high level of casework for self and others.
  • Ability to organise workload and work confidentially under pressure to meet deadlines.
  • Committed to working co-operatively as part of a team to provide a high level of service.
  • Understanding of resource management, including working with finite financial resources and the application of eligibility criteria.

  • Strong leadership skills with the ability to supervise and support team members and students.
  • The confidence to represent RBWM in multi-agency settings and make sound, person-centred decisions.
  • A proactive, flexible approach to managing change and driving service improvement.
