Job summary

East Sussex County Council - Adult Social Care currently has two vacancies to join our Mental Health Discharge Countywide team, you will work with working age and older adults and will be supported by a team that cares for you, just as much as it cares for our community.

We are looking for two registered professionals to support effective and timely hospital discharges. You will be working across East Sussex covering all Acute Hospitals for people who have been admitted to hospital to seek treatment for their mental health. This is a new enterprise seeing the position holders working as part of a remote partnership team with SPFT and VSCEs. Together with the person to identify safe, creative discharges; placing the person at the centre. You will be collaborating closely with other stakeholders, working together to achieve the utmost discharge plans.

We are currently recruiting to the following roles:

  • Social Worker(£37,575 to £43,138)
  • Occupational Therapist(£33,962 to £43,138)

Main duties of the job

We are looking for candidates who are:

  • Registered Professionals with considerable experience with adults with mental health conditions
  • Can build positive relationships with clients, family members and professionals.
  • Are self-motivated.
  • Able to meet travel requirements of the post.
  • Show a commitment to training and personal development.

Occupational Therapist

1. Undertake a professional assessment in a range of settings with individuals and to make recommendations about management of their disability and appropriate rehabilitation.

2. Advise, participate with and influence anyone involved in environment issues regarding points relating to disability and access.

3. Apply specialist knowledge and skills including advice, counselling, support, training in specific techniques, provision of equipment and adaptations and remedial treatment.

4. Advise individuals, families, carers and colleagues about the management of a condition causing disability.

Social Worker

1. Undertake a range of tasks which could include investigation of adult safeguarding issues, carrying out assessments of need, analysis and planning, assessing and managing risk, commissioning support and reviewing.

2. Identify and appropriately report children and adult safeguarding concerns.

3. Work with a range of legal interventions and departmental policies and procedures to support outcome-based planning for adults.

4. Work directly with adults in need, carers, families and their communities to support people to make informed decisions and choices.
