Main duties of the job

1. Social Worker Home Safe Hospital Discharge (Wansbeck General Hospital)

You will complete urgent needs assessments, mental capacity assessments, and work with families and partner agencies to facilitate safe discharge from hospital.

2. Social Worker West Care & Support (Hexham)

Join an integrated team supporting adults with complex physical and mental health needs. You will manage your own caseload with strong supervision and multiagency partnership working.

3. Social Worker Prevention Team (Morpeth)

Based at County Hall, you will undertake preventative, strengthsbased assessments to support adults to maintain independence. Training opportunities include AMHP and Practice Educator pathways.

4. Social Worker North Locality Care & Support (Berwick or Alnwick)

Support adults with physical disabilities and mental illness, working closely with primary care networks and local partners

Essential Requirements

  • Degree in Social Work & registration with Social Work England
  • Successful completion of ASYE
  • Valid UK driving licence and ability to travel for work
