We have a vacancy for a Social Worker in theNorth Locality Care and Support Team based in our Berwick or Alnwick District Offices.

Main duties of the job

As a Social Worker in the Care and Support Team you will work with people over 18 years of age with a mental illness or physical disability. You will work closely with the appropriate primary care teams fostering collaborative relationships which lead to better outcomes for the people we support.

Youll manage your own caseload while receiving regular supervision and consistent guidance from Advanced Social Work Practitioners and your Team Manager. A clear understanding of the Care Act and the Local Authoritys statutory duties is essential.
