Job summary
An exciting opportunity has arisen for a permanent, full time Team Manager to join the Northumberland Integrated Homesafe Service, which supports hospital discharges for patients in need of adult social care services.
We are looking for a dynamic and motivated individual to fill the role of Team Manager in our Home Safe Service on a permanent arrangement. The post will be Countywide, working alongside the existing management team you may be covering any of the hospitals based in Northumberland such as Northumbria Specialist Care Hospital, Wansbeck General Hospital, Hexham General Hospital, Blyth Community Hospital and Alnwick Infirmary.
The salary range for this full-time position is £56,233-£60,010
The post is 37 hours per week on a permanent contract basis.
Main duties of the job
As a Team Manager you will oversee and encourage best practice from our Home Safe social workers and social work support assistants. The team will be linked closely to secondary care and the core element of the role is to lead on providing excellent support to social care clients being discharge from hospital. You will be part of the larger Home Safe service covering the whole of Northumberland with strong management and peer support in the role.
The job encompasses motivating the team to high levels of performance, managing staff workload and the workflows effectively, supervision and appraisal of team members, and key other areas of work including delayed transfers of care and assisting with service development where necessary. You will also be responsible for managing a delegated health and social care budget within Home Safe.
