We are integrated with our Early Help and Social Care colleagues and share an integrated electronic record which supports communication and co-ordination.
Main duties of the job
The role of the Community Nurse is integral part of the Health Visiting team, supporting the delivery of a community public health nursing service.
You will take delegated work and be responsible for your own caseload, having a degree of autonomy over your diary and workload planning. You will be providing assessment, planning, implementation and evaluation of the Healthy Child Programme

You will be working in community settings and will hold a delegated caseload, therefore time management and prioritisation skills are essential.
