The Integrated Hospital Discharge Services purpose is to support people transferring from hospital to recover and maintain their independence and wherever possible, to return and stay in their own homes. Main duties of the job

You will be a registered care professional and have a sound understanding of relevant social care legislation and a commitment to the person centred and relationship-based practice that is the hallmark of our teams approach.

We need you to be effective, efficient and to bring creative ideas about how you will support our quality and performance

You will lead on Hospital Discharge Practice, inspiring, developing, nurturing, and supporting the way we work with people. You will lead and manage a multi-disciplinary staff group of managers and social care professionals, to achieve positive outcomes and fulfilled lives for people leaving hospital and whilst recovering post discharge. You will work with teams to enable people to achieve these outcomes working across health and social care organisations seven days a week.

As Deputy Head of Service, you will contribute to the strategic direction of the Integrated Discharge Service as well as the development of practice, procedures, and policy, both internally to Hertfordshire County Council and with system partners as we continuously develop our system approach to Hospital Discharge
