Job summary

Our ambition is providing person centred, coordinated care, enabling our residents to live independently at home for longer through an integrated, seamless continuum of services across primary care, community health services, social care, voluntary sector, and acute hospital care.

Do you have operational, strategic, and commissioning experience in either the NHS, Social Care or Care? We are looking for the ideal candidate to lead a whole system approach to Intermediate Care in Hertfordshire, working collaboratively across key organisations to create and implement an Intermediate Care Strategy.

Main duties of the job

The successful candidate will be highly skilled in leading cross organisational change, confident working autonomously, able to hold presence with senior leaders, and be comfortable with challenging environments or uncertainty.

They will require a strong focus on promoting stronger integration between local authorities, voluntary services, and the health & care system to deliver the Hertfordshire Integrated Care Strategy which makes person centred intermediate care an integrated feature of health and care pathways in Hertfordshire and West Essex.Creating coordinated, proactive services, organised in a way that makes it easier for teams to work together across different organisations and co-ordinate care around the needs of individuals. Critical to the role will be skills in commissioning, service improvement, service redesign and operational and strategic experience.

Job responsibilities

The post holder will have commissioning responsibility, provide leadership, and deliver the intermediate care strategy by transforming intermediate care, supporting patient flow, problem solving and providing challenge to system partners. A clinical background would be desirable; however, it is not essential.

Whilst developing the intermediate strategy, and the integrated intermediate care offer the post holder will work closely with other strategic leads, including discharge to assess, prevention of admission, frailty, complex care, and health inequalities. As well as creatively draw on a wide range of resources, learning from existing services and reviews, local and national research, the impending national Intermediate Care Strategy, best practice from other areas, communities and those who use our services to deliver person centred strategic priorities.
