Job summary

This is an exciting opportunity to join Hertfordshires Community Learning Disability Nursing service in Hertfordshire County Council. As a Band 6 Acute Liaison Nurse you will work within the Health Liaison Team supporting people with learning disabilities when they are admitted to a general hospital.

Main duties of the job

As an Acute Liaison Nurse, you will be working within the general hospital to offer your learning disability expertise to reduce the risks of poorer outcomes for people with learning disabilities admitted into hospital for emergency or planned admissions. Your skills in adapting and creating the right approach for the individual will help the person with learning disabilities, your clinical colleagues, and paid/family carers to get the best outcome from their hospital admission. You will be working closely with the Safeguarding Teams and Senior colleagues to develop and progress the Hospital Improvement Standards embedded into the Learning Disability Action Plan. You will be working with Junior colleagues in the team and provide leadership and clinical supervision.

Qualifications

Essential

  • Qualified Registered Nurse (at least 2 years experience post registration)
  • Some Learning disability experience
  • Ability to travel to alternative sites within reasonable timescales
  • Multi-disciplinary Team working experience
  • Able to cite evidence-based practice

Desirable

  • Car driver, with access to a car
  • Experience as a nurse supervisor
  • Community-based health care experience
  • Knowledge of social model of disability
  • Teaching skills
