Adult Disability Service - East

The Adult Disability Service provide support to adults with a learning or physical disability. We promote personalization through our assessment process focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by offering personal budgets to enable people to direct their own support.

As a CCO in this multi-disciplinary team you will be responding to those in need of social care support and undertaking the required work in accordance with the Care Act 2014 and Mental Capacity Act 2005.

Main duties of the job

We are a service motivated by and dedicated to social care intervention and community social work. Our core aim is to support people to be influential and active members of their community and promote true citizenship in line with our Connected Lives model of care.

We need CCOs that can engage with people; promoting and enabling inclusive community options as well as supporting a self-directed approach and making the most of universal services. We want our CCOs to be resilient, robust as well as imaginative and reflective. We are a small team and we work cohesively to achieve our outcomes and we are committed to smart/flexible working.

The team is community based and it would be beneficial for applicants to be holding a full UK driving licence.

Hospital

  • Princess Alexandra Hospital Team- Harlow
  • Lister Integrated Discharge Team - Stevenage
  • Community Hospital Team - East & North Herts

You will work as part of a busy social care team at Princess Alexandra Hospital or you will work as part of a busy integrated hospital discharge team at Lister Hospital, completing social care assessments to ensure that people are discharged in a safe and timely manner. You will develop close working relationships with the hospitals Discharge Team and care providers to ensure effective care planning. You will adhere to Hertfordshires Connected Lives Practice Principles by ensuring that people are supported to maintain their independence and receive person-centred care and support.

This is an exciting opportunity to join a high performing and supportive team working to support citizens over 65 years who live in a vibrant, multi-cultural town. We are committed to working in collaboration with our key partner agencies (health, social care and mental health services). The aim of this role to promote independence by assessing older people to identify their desired outcomes to maximise independent living whenever possible.

You will undertake assessments of need to determine eligibility and implement care and support plans with clear outcomes to achieve or maintain well-being. You will keep the service user at the centre of all activity and address assessed needs within the legislative framework. You will need to be able to work autonomously within agreed timeframes and manage frequently changing situations. You will work as part of an integrated team, attend multi-disciplinary meetings and need to have excellent communication skills.
