Senior Enablement Therapist - Discharge to Assess

Job summary
As a valued member of the multi-disciplinary team, you will work in partnership with people following hospital discharge as they transfer to care home setting with the goal of returning to their own home in the community. You will undertake specialist assessments to identify person-centred outcomes, supported by goal setting and therapy plans. In this role you will promote independence and participation, through meaningful occupation and a strengths-based approach. You will support the discharge home form the care home and follow up with enabling interventions in the community. The intervention is time-limited, generally between 4 - 6 weeks, but can be extended up to 12 weeks where required. The care management is element is completed outside of the enablement team. Being part of the Discharge to Assess process you will liaise with colleagues across all sectors to inform the appropriate level of support in the community and will be managing a caseload ensuring a high standard of therapy provision. You will work collaboratively alongside social care colleagues and health teams within the community.
Main duties of the job
This role involves responsibilities such as:
- Promoting and supporting a strength-based approach to therapy intervention.
- Undertaking assessments and goal setting with individuals and their carers.
- Designing personalised programmes that enable, and encourage the individual to relearn skills and regain abilities
- Maximising independence, health, and well-being.
- Reviews of moving and handling, provision of specialist equipment, & minor adaptations to support identified goals.
