Job summary
Our Connected Lives Gateway Service works with adults to prevent, reduce and delay need by ensuring people access a wide range of information, advice and support at the right time and in the most straight forward way possible.
We are recruiting Social Workers and Occupational Therapists to support the Gateway Service to deliver this vision as part of our plans to expand our prevention and enablement workforce in line with ourConnected Lives practice modelfor adult social care.
The new Gateway Service strives to provide the warmest welcome for everyone. We have listened to our residents and staff and developed a new Gateway Service with prevention and enablement being at heart of everything we do. To us, enablement doesnt just mean enabling-style homecare or bed-based commissioned enablement services, in Hertfordshire enablement is an approach and culture, designed to support peoples wellbeing in a truly person-centred way.
We are extending the time that workers can spend supporting people to take positive risks and live the life they want through an enhanced enablement offer. Successful enablement does not always mean no formal care at its conclusion.
We are increasing our qualified Occupational Therapy and Social Work roles because we know that by having a long-term plan this will help us to meet increasing demands and will in turn create more fulfilling roles, manageable caseloads and improved staff wellbeing.
Main duties of the job
You will need to be a confident individual, capable of working independently, enabling people to live independently in the community. You will conduct holistic assessments and interventions could include minor and major adaptations to achieve desired outcomes. We encourage smart working with the benefit of a laptop and smart phone, so you have the flexibility to work at home or at one of our touchdowns in the area.
You will be an ambassador for our HCC Values, Behaviours, and Professional Standards, both internally and externally. This will also involve contributing to the learning and development of others, including mentoring and supervising. Alongside this, we will continually support you in your professional development.
