Job summary

Would you like to make a real difference to people with learning disabilities and their carers through working in an award-winning community nursing service? If you do this is your chance to be involved with an innovative and progressive nursing service with a range of nursing roles and opportunities for career progression.

We are seeking a Nurse to support the Welwyn and Hatfield Adult Disability Team for a 12 month maternity leave cover. You will be working within a health and social care setting and supporting health partners such as GPs and other health professionals to improve the health of people with disabilities.

You will develop relationships with wider health partners to address the health inequalities for people with learning disabilities identified through the LeDeR Reports, and increase the uptake of health initiatives by engaging people with learning disabilities and their carers. You will have access to a range of training and on the job training to enable you to develop as a practitioner and take part in projects with partners such as NHSE.

Applicants must have knowledge of the requirements and challenges of community nursing and be able to demonstrate transferrable skills from other nursing environments if you have not had community experience. You must be registered as a Nurse with a valid NMC registration and if your registration is not in Learning Disability Nursing be able to demonstrate experience, skills and knowledge in working in this area.

Main duties of the job

We are based in Stevenage however we encourage smart working with the benefit of a laptop and smart phone. You would be required to be office based 40% of the week and you have the option of working at home or at one of our touchdowns across the area. Job responsibilities

Purpose of the Job

The Locality teams offer an integrated specialist health and social care service to people with a learning disability, living in the community. You will take an active role in the day-to-day service delivery of the Adult Disability Team and ensure that a high quality community learning disability nursing service is effectively delivered to the community/locality in line with necessary targeted timescales. You will actively take part in duty rota systems and team business and referral meetings.

Main Areas of Responsibility

To work as part of a Multi-Disciplinary Team offering a Nursing service to people with learning disability who are receiving care from Primary, Secondary or Tertiary Health services.

To work closely with nurses and other professionals to ensure that people with learning disabilities receive equitable healthcare from generic health services.

To work closely with health and social care colleagues across all organisations to ensure that Learning Disability Mortality review (LeDeR) evidenced inequalities are addressed.

Clinical

To undertake initial health assessments if new nursing referrals are identified in the integrated referral process.

To carry a caseload and operate as an allocated nurse to service users referred to the learning disability nursing service.

To be part of the Nursing Triage processes and undertake the necessary nursing assessments for those people who are eligible to receive a nursing service.

To carry out joint assessments with other Multi-Disciplinary Team members.

To give clinical advice regarding medication in an understandable way to people who use the service and family/paid carers, and to liaise with pharmacy according to the individuals needs and abilities

To participate in project work in the service as necessary (e.g., task and finish groups and health promotion work))

To liaise with wards and departments within the acute hospitals, working closely with the Acute Liaison Nursing colleagues.

To liaise with Primary Care Networks to work alongside GP Practices to reduce health inequalities for patients on the learning disability register.

Leadership

To support unqualified nursing staff in their continuing professional development

To provide professional advice and support to the unqualified nursing staff, in consultation with the Band 6 community nurse

Practice Development

To maintain an up-to-date knowledge of causation, treatment and care, and professional developments by reading, publications and attending appropriate training, and to exchange this knowledge with other members of the team.

To participate in quality improvements such as the Nursing Audits and respond to feedback from people who use the service.

To support the implementation of clinical standards and new practices.

To work with the Multi-disciplinary team to ensure best practice.

To increase knowledge and research and maintain a questioning and analytical approach to care.

To participate in Practice Governance work.

To share practice issues and lessons learnt with nursing staff and other team members.

Patient and User Involvement

To provide advice and support to service users, families and carers.

To promote the involvement of the people who use the service and paid/family carers in all aspects of their care.

Managing Risk

To participate in effective risk management systems such as Triage Referral management systems.

To work within the framework of health and safety and of Infection prevention and control Policies.

Education and Training

To assist in facilitating student nurse placements within the Community Learning Disability Nursing service.

To assist in facilitating the learning of skills to service users, carers and professionals on an individual or group basis, as appropriate
