Job summary
As a registered Social Worker in the Hampshire County Council Continuing Healthcare (CHC) team, youll work within a multidisciplinary environment collaboratively with your social care colleagues and other professionals in the varied and complex field of NHS CHC. Youll be supported to manage a diverse caseload, working across all care groups including older and younger adults, with mental health concerns and physical and learning disabilities.
Main duties of the job
What youll be doing:
- Supporting individuals with their onward care journey following a Discharge to Assess period from hospital.
- Undertaking assessments in line with the Care Act 2014.
- Engaging with multidisciplinary teams to support completion of NHS CHC assessments.
- Collaborating with individuals and their families to transition them to suitable care provision following assessment.
- Undertaking mental capacity assessments.
- Managing challenging and complex cases.
What were looking for:
- Social Work qualification and registered with Social Work England (SWE). We would welcome applicants wanting to complete their ASYE.
- UK based experience within Adult Social Care.
- Experience and / or knowledge of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care July 2022 (Revised).
- A sound understanding of how to apply a strengths-based approach.
- Understanding of social work issues, legal frameworks and social services statutory duties.
- Commitment to demonstrating our values and behaviours.
About us
This is a hybrid role (home and office working) with your office base being the closest Hampshire County Council office to your home address. This role requires travel across Hampshire and working 20% of your week in office. The team meets once a month in Basingstoke.
