Community Reablement Assistant

Job summary

When someone leaves hospital, everyday tasks can feel overwhelming. As a Community Reablement Assistant, youll help people regain their confidence and independence in their own homes - supporting them to live life as fully and independently as possible.

Our Rapid Response service is available from 07:00 to 22:00, every day of the year, enabling us to deliver flexible, timely support whenever its needed most. Youll work on a fixed 3-week rolling rota, which includes a mix of early and late shifts across weekdays, weekends, and public holidays.

Main duties of the job

Youll cover one of four areas:

  • Central Hampshire (including Winchester, Bishops Waltham, Andover, Hedge End, Locks Heath and West End) working 18.5 hours per week.
  • North Hampshire (including Basingstoke, Alton, Aldershot, Liphook, Whitchurch, Bordon and Farnborough) working 18.5 hours per week.
  • South Hampshire (including Fareham, Gosport, Havant, Petersfield, Horndean, Emsworth, Hayling Island and Portchester) working 18.5 or 25 hours per week.
  • West Hampshire (including New Forest, Eastleigh, Totton, Hythe and Romsey) working 18.5 or 25 hours per week.

As the role involves visiting people in their own homes, youll be required to travel to various locations within Hampshire and its essential that you have a current drivers licence and access to a reliable vehicle on a regular basis. Youll be able to claim your business travel mileage to and from your home address in line with our expenses policy.
