Job summary

As part of a multi-disciplinary team, youll be based at Clarence Unit in Gosport, our short-stay Recovery and Assessment suite, for a minimum of three days a week so you can meet with individuals and their families. You may also be needed occasionally to support onsite at Queen Alexander Hospital in Portsmouth. As a qualified, registered Social Worker youll be confident completing assessments and developing plans for onward care, managing multiple cases at a time. Youll enjoy building strong relationships with colleagues and collaborating with a range of other professionals, using your expertise to achieve timely onward moves.

As no two cases are the same, youll enjoy a diverse caseload and have the opportunity to really stretch your social work skills. Dealing with a broad spectrum of complex issues, your sound understanding of our legal obligations will support your decision making.

Youll use a strengths-based approach to make confident decisions, even where there is uncertainty. With an understanding of how our services are structured, youll really enjoy using a proactive problem-solving approach to achieve successful outcomes.

We welcome applications from Newly Qualified Social Workers, as we can support you through your Assessed and Supported Year of Employment. However, candidates must have graduated and be registered with Social Work England before applying.

Main duties of the job

The Clarence Unit is a Short Term Service. People can stay at the Unit for up to 28 days, where they work to improve their confidence, work on mobility and re-establish their independence after a discharge from hospital. The social work team work on an onward plan for when someone is ready to leave the Unit. This involves assessing needs under the Care Act 2014 and working through any issues affecting their next move. Our aim is that people return to their own home wherever possible. Some individuals may require ongoing residential or nursing care.

Your role

As a Social Worker, you'll be part of a supportive and friendly team working with other social/health care professionals to support with hospital discharges and moving people to short term care services and onward care providers. You'll complete assessments, develop care plans, and provide people leaving hospital with support and guidance to enable them to maximise their wellbeing. Through your person-centred approach, youll focus on the elements of care, support and treatment that matter most to the person, their families and carers.

We offer the opportunity to progress to Senior Social Worker roles through our Excellent Practice Validation programme.
