Job summary

As part of the continued growth and development of the Reablement Service within Hampshire, we are excited to share the opportunity for you to join us in the position of an Advanced Practitioner Occupational Therapist. This role will be fundamental to support our evolution into an Occupational Therapy principled service.As an Advanced Practitioner (OT), you will be a practice leader who champions excellent Occupational Therapy evidence-based practice within Reablement, Hampshire County Council, external organisations and with other professionals.You will have the skills, knowledge and experience to provide practice expertise within your own complex caseload and you will guide, advise and supervise / mentor team members, with an expectation to provide support on a countywide basis. Your practice will be characterised by a high degree of autonomy and complex decision making and you will be a key player within both local leadership teams and the countywide consultant team.Advanced Practitioners in Occupational Therapy will support excellent standards of practice across both the Reablement Service and the wider department. This is encompassed by the four pillars of professional practice, facilitation of learning, leadership and education, research & development. (RCOT 2021).The Reablement Service operates 7 days a week and there will be a requirement to work weekends on a rotational 1:8 basis.

Main duties of the job

To provide person centred functional analysis and interventions, through the enactment of social care legislation and local government requirements. This is informed by occupational therapy clinical knowledge, skills and values. There is particular responsibility to work as an independent practitioner in high risk cases, manage workload, and/or line manage/supervise staff.

Risk Management:

  • Undertake and/or support staff in the assessment of risk and apply clinical reasoning in challenging situations. Apply strategies to mitigate risk and identify least restrictive interventions
  • Undertake and/or support staff in the assessment, planning and review for safeguarding in high risk cases
  • Complete and/or support staff in the completion of functional assessments and reviews that will identify risks, needs, options, and promote well-being and maximise independence
  • Manage and/or support staff in high risk cases and/or situations, and ensure occupational therapy interventions are proportionate to need
  • Model and support staff with effective workload management skills
  • Report concerns and complaints and learn from them in reflective practice

  • Provide specialist information and advice using a professional understanding in the application of legislation and guidance and ensure the staff and/or services you supervise provide the same
  • Provide and ensure the staff and/or services you supervise, provide information and advice to local community networks, that support local empowerment and well-being and maximise independence. This will include implementing new ways of working
  • Implement innovative ways of enabling information and advice to be disseminated by other professional colleagues and teams, that address social care needs
  • Empower and advocate on behalf of individuals and communities in challenging situations, and enable access to independent advocates when appropriate.

Multidisciplinary Working:

  • Actively build and maintain and/or support staff, in building relationships with other professionals, and pursue opportunities to work collaboratively with multidisciplinary teams
  • Actively Identify cases that will benefit from planned multi-disciplinary working, and/ or an appropriate response to unplanned work.
  • Take the lead for high risk multi-disciplinary case work as appropriate, and/or support staff in multi-disciplinary case work
