In Gateway to Care we
provide a person-centred approach to support people to manage their health and
wellbeing. This is so they can maximise their independence, choice, and control
to live healthier lives and where possible, to reduce future needs for care and
support. We provide information and advice to those people who may need a level
of support that is within the remit of adult social care.

Main duties of the job
Your new role
You will be mainly office based to support your team at The Shay Stadium, in Halifax. You will have a joint responsibility with the Team Manager for staff within the team ensuring performance management, regulatory compliance, and service delivery, ensuring service priorities are met and take responsibility as required in the absence of Team Manager. You will be a proactive worker who has a proven record of working under pressure and supporting other staff members.
Your responsibilities will also include:
- Working collaboratively across the Council, with partners and statutory and voluntary agencies, to ensure a joined-up approach.
- Line management of social workers and Social Care Advisors.
- Developing a high level of expertise in particular areas of practice, and leading on those areas
About us
To apply, you need to
be a qualified Experienced Occupational Therapist or Social Worker with current
Social Work England registration and with significant post-qualification
experience and knowledge of the Care Act 2014. You will also hold an additional
post qualifying qualification such a Practice Education, Best Interest Assessor
and/or Approved Mental Health Professional or be willing to work towards this
within a specified time frame.

Job responsibilities
1. Ensure that effective systems are in place to screen referrals against eligibility criteria. To work
with partners to ensure that referrals are dealt with by the creative deployment of resources
both in terms of staffing and other options.
2. Prioritise and allocate work to appropriate staff based on the complexity and urgency of a
service request. To utilise the resources of the whole team and other skills within the overall
service.
3. Ensure in conjunction with the Team Manager the effective management and deployment of
resources within budgets to meet agreed policy objectives and service priorities. To effectively
communicate with the team about the management of budgets.
4. Responsible for ensuring that the review and evaluation of Support Planning is carried out in
accordance with practice guidelines, and that appropriate adjustments are made to Support
Plans to reflect changing needs.
5. Responsible for ensuring a systematic approach to the assessment planning, implementation
and review of health needs in accordance with policies and procedures.
6. Ensure that staff have fully informed and involved users and carers in the process of
assessment and in agreeing personal budgets, packages of care, and to ensure copies of
assessment and care plans have been provided by staff.
7. Promote the professional development and wellbeing of individual members of staff through
regular supervision, identify training opportunities, work in accordance with agreed outcomes
in Shared Conversations, interviews and assist qualified professionals to maintain their
professional registration.
8. Responsible for own professional development in accordance with professional registration
requirements as a qualified Social Worker.
9. Assist the Team Manager in ensuring that the teams work is adequately covered in staff
absence and to take responsibility, as required, for covering the work of the team manager in
their absence and to monitor/manage sickness absence in accordance with Council policies.
10. Support the Team Manager with the operational management of the team and to positively
address issues of resource management. To assist in the monitoring of budgets in order that
future expenditure is projected and planned for and that decisions made are firmly embedded
in awareness of resources.
11. The post holder will support digital engagement and share expertise to build capability across
the wider organisation and with external partners.
12. To work collaboratively with the Advanced Practitioner in the learning and development of the
team to promote, develop and maintain both qualified and differently qualified staff providing
guidance and leadership in promoting, maintaining and developing the skills and professional
requirements of their roles.
13. Assist the team manager in formulating pathways and service delivery to ensure the best
outcome possible for the individual. Being mindful of budgetary limitations and legislative
requirements.
