We're looking for a passionate, forward-thinking Social Worker to join our dynamic IntegratedAdult Social Care Team in Okehampton and Moretonhampstead.

Main duties of the job

What you'll be doing

You'll work with individuals aged 17+ transitioning into adult services, supporting those with physical and learning disabilities, long-term health conditions, and safeguarding needs. Using a strengths-based approach, you'll empower people to live independently and safely in their communities. You'll be at the heart of person-centred care, using your professional expertise to assess needs under the Care Act, support decision-making in line with the Mental Capacity Act, and protect adults at risk. Your relationship-building skills will shine. This is for 30 hours per week, days to be discussed but ideally to include a Friday.
