Job summary

Social Care Reablement (SCR) is a short term intervention service, supporting people to regain their independence through working towards personal goals in their own home.

We are looking to recruit a Reablement Team Leader to enable the service to support individuals through the reablement process.

Main duties of the job

Working as part of a rota currently covering from 8am to 6pm and 1 in 4 Saturdays you would participate in covering the duty desk for incoming referrals and queries, manage a client caseload through their service and supervise a team of Reablement Support Workers.

The role also includes management of a web-based rota system and electronic care recording systems. The successful candidates will have excellent communication skills, effective ICT skills and the ability to assess & set and goals.

The objectives are to support people, who need help because of age, disability or personal circumstances, to enable them to remain in their own homes with as much independence as possible.

To complete the Goal Planning assessment process and monitor and review the delivery of the goals set for an individual both during and at the end of the reablement term.

To ensure that all activities undertaken by the Support Worker - Reablement individual, carer or other people involved follow the Policies and Procedures laid down nationally and locally.

Qualifications

Essential

  • NVQ level 2 in Health & Social Care (or willingness to gain this) together
  • with other learning as required to meet individual user and service requirements.
  • Knowledge of Health & Safety Regulations.
  • Commitment to on going continuous development & training as required
  • by the service.

Desirable

  • NVQ level 3 in health & Social Care or equivalent.
  • Any other appropriate management qualification.

Experience

Essential

  • Previous community experience of hands-on care in the health and social care environment, in public, independent or voluntary sector.
  • An awareness of the needs of those individuals likely to need the
  • reablement service.
  • Understanding of Risk Assessments.

Desirable

  • Experience in the provision of Social Care Reablement service.
  • Managing financial budgets.
  • Previous experience of working in a multi disciplinary team.
  • Experience of report writing.
