We are seeking a skilled and motivated Occupational Therapist to join our Therapy Services Team within the Adult Social Care Department. In this role, you will be responsible for assessing, planning, and implementing occupational therapy interventions to support adults with various needs and long-term conditions, to enable people to live at home longer and to take charge of their life.
Main duties of the job
Key Responsibilities:
- Conduct comprehensive assessments of peoples physical, emotional, and social needs.
- Develop and implement individualised goal setting to promote independence and improve quality of life.
- Provide therapeutic interventions, including activities of daily living, mobility, and cognitive rehabilitation.
- Collaborate with multidisciplinary teams to ensure coordinated care and support.
- Monitor and evaluate a persons progress, adjusting goals settingas necessary.
- Maintain accurate and up-to-date documentation of assessments, goal setting, and progress.
- Educate people and their families about occupational therapy techniques and strategies.
