We’re looking for a dedicated Social Worker to join our dynamic and compassionate Older Adult Community Mental Health Team in Bath and North-East Somerset (BaNES). If you're an experienced practitioner, this is your opportunity to be part of something meaningful.
About the Team
Our team is part of a recently transformed service aligned with the Community Mental Health Framework. We support older adults experiencing mental ill health and complex neurodegenerative conditions, helping them live safely and independently.
We are a truly integrated, multi-disciplinary team made up of professionals from both the local authority and the Avon and Wiltshire Mental Health Partnership (AWP). You’ll work alongside nurses, psychologists, medics, allied health professionals, and support workers in a collaborative, co-located environment.

About the Role
Flexibility to work across both locations and the wider BaNES area is essential.
As a Social Worker, you will:
- Manage a varied caseload, including complex and higher-risk cases
- Undertake assessments of need and risk, and develop strengths-based, outcome-focused care plans
- Lead safeguarding enquiries and contribute to multi-agency decision-making
- Work collaboratively across health and social care to deliver high-quality, person-centred support
About You
We’re looking for someone who is:
- A registered Social Worker with Social Work England
- Minimum of at least 12 months post qualifying experience.
- Experienced in working with individuals with complex mental health needs
- Knowledgeable in adult social care, safeguarding, and mental health legislation
- A strong communicator with excellent written and verbal skills
- Committed to anti-discriminatory practice and person-centred care
- You’ll need a full driving licence and the ability to travel across the area.
