This is an exciting opportunity to join our Specialist Adult Social Care team in Keynsham as a Transitions Social Worker. You’ll play a vital role in supporting young people aged 16 to 25 as they move into adulthood, helping them navigate complex needs and access the right support at the right time.

You’ll be part of a warm, collaborative, and forward-thinking team, working alongside a wide network of professionals including CAMHS, SEND, Children’s Services, AWP Mental Health Teams, and more. It’s a chance to make a real difference in the lives of young people and their families.

  • A Full UK Driving Licence and access to a vehicle
