As a Registered Nurse (Band 5), you'll be an integral part of our Primary Care team, supporting some of our most vulnerable patients and planning care for people with complex and challenging health needs. You'll be confident in all aspects of primary care delivery - including undertaking health assessments and performing diagnostic tests - and able to work flexibly to meet the needs of our service.

Main duties of the job

  • Working closely with our management team and key stakeholders, you will contribute to the delivery of a comprehensive primary care health service.
  • You will be part of a multi-disciplinary team, undertaking physical health assessments of patients, planning care and providing a range of health treatments/interventions.
  • You will support patients to address issues that affect their own health, behaviour and social welfare
  • Manage competing priorities and react to unforeseen circumstances, incidents and emergency situations.
