Job summary
This is a great opportunity to build your skills across Occupational Therapy and Physiotherapy while supporting people to stay safe and independent at home. You'll work as part of a supportive multidisciplinary team, including Occupational Therapists, Physiotherapists and other Associate Practitioners. Together, you'll help people with physical or neurological conditions reach their goals and improve their daily lives. You'll be supported through supervision and clear competencies as you grow in the role.
Main duties of the job
What you'll do:
- Carry out holistic assessments in people's homes
- Plan and deliver personalised rehabilitation based on agreed goals
- Support people to practise daily activities and improve mobility
- Recommend adaptive equipment to help people live safely
- Work with families, carers and external agencies to coordinate support
What we're looking for: - Experience working in a health or social care setting
- Skills in supporting people with physical or neurological needs
- Ability to manage your own caseload with guidance when needed
- Confidence travelling to visit people in their homes
- A caring approach and willingness to learn across two disciplines
