Job summary

This is a great opportunity to build your skills in community nursing while supporting people to stay well at home. You'll work within an integrated team that provides safe, person-centred care to people with a wide range of health needs. You'll be part of a supportive group of colleagues who work closely with other health and social care partners to offer joined-up care.

Main duties of the job

What you'll do:

  • Deliver high-quality, holistic nursing care to people in their own homes
  • Work closely with Registered Nurses and wider professionals to plan and review care
  • Carry out clinical tasks in line with your Nursing Associate training
  • Build strong, trusting relationships with people, their families and carers
  • Keep accurate records using our Electronic Patient Record (EPR) system
What we're looking for:
  • A qualified Nursing Associate with current registration
  • Someone who can work well within a team and use their own initiative
  • Clear communication skills and a caring, professional approach
  • Willingness to learn, with full training and support provided
  • Ability to travel within the local area using suitable transport
