Job summary
This is a great opportunity to build skills in lifestyle support while helping people make positive, lasting changes to their health. You'll work closely with colleagues across the Wellbeing Service to offer safe, personalised support that empowers people to become more active and improve their wellbeing. You'll be part of a friendly, multi-disciplinary team that includes health improvement specialists and partners across health, social care and the community. Together, you'll help people understand their choices, set realistic goals and stay motivated throughout their journey. What you'll do:
- Carry out one-to-one consultations to assess needs and create personal action plans
- Lead safe and engaging physical activity sessions for people with a range of health conditions
- Support clients to stay motivated through regular reviews and long-term guidance
- Work with partners to ensure smooth referrals and joined-up support
- Keep accurate records to help evaluate and improve the service
Main duties of the job
What we're looking for:
- A recognised GP referral or Level 3 physical activity qualification
- Experience supporting people to change their behaviour in a supportive, non-judgemental way
- Skills in leading physical activity sessions for different needs and abilities
- Confidence working independently and as part of a wider team
- Good communication skills and an understanding of behaviour change approaches
