Job summary
Working with our Community Nursing team, you'll provide high-quality care to people in their own homes, helping them stay safe and independent. Your role will make a real difference to patients and families by supporting their health and wellbeing. You'll be part of a supportive team that values collaboration and professional development. Main duties of the job
What you'll do:
- Carry out holistic assessments and plan individualised care
- Deliver nursing interventions such as wound care, medication administration and health education
- Communicate effectively with patients, families and partner agencies
- Maintain accurate records in line with professional standards
- Contribute to service improvements and uphold CHCP policies
What we're looking for: - Registered Nurse (or working towards final assessment)
- Understanding of community nursing and evidence-based practice
- Ability to work independently and as part of a team
- Basic IT skills and willingness to learn
- Flexible approach to meet service needs, including occasional weekends and bank holidays
