Job summary

Working with our Community Nursing team, you'll provide high-quality care to people in their own homes, helping them stay safe and independent. Your role will make a real difference to patients and families by supporting their health and wellbeing. You'll be part of a supportive team that values collaboration and professional development. We'll help you build your skills while delivering care that reflects compassion, dignity and respect.

Main duties of the job

What you'll do:

  • Carry out holistic assessments and plan individualised care
  • Deliver nursing interventions such as wound care, medication administration and health education
  • Communicate effectively with patients, families and partner agencies
  • Maintain accurate records in line with professional standards
  • Contribute to service improvements and uphold CHCP policies
What we're looking for:
  • Registered Nurse (or working towards final assessment)
  • Understanding of community nursing and evidence-based practice
  • Ability to work independently and as part of a team
  • Basic IT skills and willingness to learn
  • Flexible approach to meet service needs, including occasional weekends and bank holidays
