Job summary
We have an exciting opportunity for a Registered Nurse (Band 5) to join the North Hull Nursing and Conditions Community team.
We are looking for an enthusiastic and self-motivated Registered Nurse to work within our community nursing team, providing holistic high quality nursing care to a dedicated client group. You'll support people in their own homes by delivering safe, effective nursing care that helps them stay well and independent.
Working as part of our district nursing team, you'll assess needs, plan care and provide treatments that make a real difference to patients and their families. You'll collaborate with colleagues and partner agencies to ensure care is joined-up and responsive.
Main duties of the job
What you'll do:
- Carry out holistic assessments and create individual care plans
- Deliver nursing interventions such as wound care, injections and medication management
- Communicate clearly with patients, families and multi-disciplinary teams
- Maintain accurate records and follow professional standards
- Support service improvements and mentor students or junior staff
