Clinical Pharmacist (Chronic Pain Management)

Job summary

CHCP's Community Chronic Pain Management Service is looking for a temporary part-time Clinical Pharmacist to join our enthusiastic and progressive team who provide an extensive service across Hull and the East Riding of Yorkshire.

CHCP's Chronic Pain Management Service is committed to providing the highest quality of holistic care working in partnership with patients, their care givers and other health care providers.

Main duties of the job

You will work as an integrated member of a multi-disciplinary team supporting service delivery of individualised care plans for patients in a variety of health care settings. Such care plans will primarily be around the optimisation and de escalation of pain medications, particularly for patients taking high levels of opioids and gabapentinoids.

The post is a temporary 6 month contract for 19.5 hours per week, which may be worked flexibly across Mon-Fri, 9am-5pm.

  • Masters degree Pharmacy (MPharm) or equivalent pre 2000 qualification, plus one year's pre-registration training
  • GPhC Registered Pharmacist
  • Post-graduate training in clinical or pharmacy practice to diploma level or working towards a diploma level qualification or able to demonstrate equivalent experience
  • Evidence of Continuing Professional Development
  • Supplementary or Independent prescribing qualification

Knowledge

Essential

  • Able to apply clinical knowledge appropriately in a variety of practice settings or patient groups
  • Understanding the current national policies and legislation relating to medicines
  • Thorough working knowledge and understanding of primary care and medicines management issues
  • Good analytical and decision-making skills around medicines information
  • Computer skills e.g. Microsoft Word, Excel, PowerPoint, e-mail

  • Familiarity with SystmOne and other GP computer systems
