Community Link Worker

Job summary

The focus of the role will be to refer and signpost to the correct services to support early intervention and prevention of reliance on more specialist services, while reducing pressure on GP practices. Referrals willbe received from primary care health and social care professionals aligned to the practices, as well as external organisations and patient self-referrals. Individuals can be referred to the Community Link

Main duties of the job

Workers for a variety of reasons including but not limited to social issues, social isolation, housing instability, debt/financial hardship, education/employment matters and low-level mental health needs.Referred patients may present with addiction/substance misuse/smoking alongside other issues that can impact negatively on both lifestyle behaviours and overall health and wellbeing.

A full UK Drivers license and a vehicle will be required as you will be expected to travel across several different bases.

Qualifications

Essential

  • Level 3 in Health and Social Care (or working towards an equivalent qualification) or demonstrable equivalent experience
  • Completion of Social Prescribing e-learning programme or willingness to complete this as part of induction

Desirable

  • Diploma in a relevant subject area
  • Clait, ECDL or equivalent IT qualification
  • Training in motivational coaching and interviewing or equivalent experience

Knowledge

Essential

  • Significant understanding of the role of social prescribing within the wider health and social care sector
  • An understanding of the role of voluntary and community sector organisations and the contributions they make to the health and wellbeing of individuals
  • Knowledge of local communities including resources and assets
  • Knowledge of factors influencing behaviour and behaviour change approaches
  • An understanding of health and social care infrastructure and how services are provided to the local population
  • An understanding of the factors that can impact on an individual's health and wellbeing
  • An understanding of community development and assetbased approaches
  • An understanding of the barriers and challenges that people may face in connecting to and accessing local activities and support
  • An understanding of the importance of accurate record keeping and knowledge of information governance requirements
  • Knowledge of Microsoft office such as Outlook, Excel, Word and Power Point

  • Experience of peer support and/or mentoring others

  • Experience of collating and presenting information
  • Experience of involvement in service monitoring and evaluation
