Job summary

An exciting opportunity for has arisen to join the CHCP Volunteer Hub team, we are pleased to advise we have a vacancy for a Empower Health Tutor with specific responsibility for delivering the EMPOWER Programme, the Chronic Disease Self-Management Programme.

Main duties of the job

As a Health Tutor you will be required to work within the local community delivering the programme to groups. You will possess excellent communication skills in order to motivate and support them in making positive changes to their lifestyles.

  • Level 3 qualification in Business Administration/Health and Social Care or equivalent level of knowledge, skills and experience
  • Required to attend the EMPOWER 6 week course as a participant followed by a tutor training course with (SMRC) Self Management Resource Centre

Desirable

  • Supervisory management qualification
  • International Computer Driving License (ICDL)

Knowledge

Essential

  • A detailed understanding of General Practice office environment
  • IT skills

Desirable

  • An understanding of the practice clinical systems
  • Working knowledge of staff motivation techniques

Experience

  • Practical experience of working with others
  • Experience of using one's own initiative
  • Experience of working within a General Practice Office
  • Practical experience of computerised recording systems
