An exciting opportunity for has arisen to join the CHCP Volunteer Hub team, we are pleased to advise we have a vacancy for a Empower Health Tutor with specific responsibility for delivering the EMPOWER Programme, the Chronic Disease Self-Management Programme.
Main duties of the job
As a Health Tutor you will be required to work within the local community delivering the programme to groups. You will possess excellent communication skills in order to motivate and support them in making positive changes to their lifestyles.
