Job summary

Jean Bishop Frailty Community Services (JB-CFS) have been operational since May 2018, and in that time provided innovative, anticipatory patient tailored care to frail residents of Hull and the surrounding areas, including those residing in care homes.

The Centre has already been recognized for national awards and continues to receive outstanding patient feedback. The JB-CFS also featured on NHS England's website dedicated to the NHS Long Term Plan as the leading case study in the Ageing Well section. More recently, the team featured on a Panorama programme which assessed the innovative and new ways of working delivered by our team, evidence to support not only our frail population but also the demands placed on the NHS.

Main duties of the job

The team working at the JB-CFS feel privilege to be a part of delivering transformed care for older people in Hull. We are currently seeking a Band 5 - Band 6 Physiotherapist for a newly created development role. The post holder will have a knowledge of evidence-based Physiotherapy practice, you will be working as part of a multi-disciplinary/multi-agency team and will be responsible for own autonomous practice and ensure effective patient/client care is maintained.

This will necessitate you visiting patients within their own homes for the delivery of Physiotherapy interventions. You will be offered supervision on a regular basis and will be able to network with other Physiotherapists in our Professional Development group. There will also be opportunities to work with other work streams such as the Advice and Guidance Support and the Care Homes Team.

This is an amazing opportunity for a Physiotherapist who also has genuine interest and empathy for older and frail people.

Essential

  • Professional Physiotherapy qualification to degree level or equivalent.
  • Current full HCPC Registration

Desirable

  • Develop leadership, training/presenting experience qualification supported by CPD courses
  • Post-graduate training in relevant area
  • Demonstratable experience as a Practice Educator of preregistration students

Knowledge

Essential

  • Able to justify clinical decisions to other Medical Staff, colleagues and other professionals
  • An understanding of relevant clinical practice/standards within identified clinical area
  • Competent in skills appropriate to the post e.g. Manual Therapy/Neuro
  • Basic IT skills
  • Evidence of reflective practice, IST and CPD portfolio
  • Autonomous practitioner works within codes of practice, policies and guidelines.
  • Accountable for own professional action
  • A broad understanding of relevant policy, drivers and their application to clinical and service area

Desirable
