Job summary

We have an exciting opportunity for a band 5 Registered nurse to join the community-based Nurse led Anticoagulation Team. We see patients with a Hull GP and a small cohort of patient's from the East Riding area (Goole).

The role will involve helping patients to manage their Anticoagulation safely within the community, undertaking home visits within the services geographical area and managing the day to day running of clinics citywide. You will be providing support to patients, carers, junior members of the team, and the clinical team leader.

You will work within the guidelines of local and national protocols to ensure you maintain the safety and wellbeing of all our patients. We are looking for enthusiastic, proactive nurses to provide holistic high quality nursing care to a dedicated client group.

We provide a 7 day service, with working hours between 08:00 - 17:00 or09:00 -18:00 Mon to Fri, and 09:00 - 13:00 on a Sat and Sun.

Main duties of the job

The successful applicant will need to be flexible, a good team worker, used to working under pressure, have excellent interpersonal skills and be able to communicate effectively with patients, carers and other members of the MDT. You will need to have an adaptable approach to work in order to respond to the dynamic changes that can occur within the service on a day to day basis.

Additionally, you will need to demonstrate IT literacy as you will be using a decision support software system to manage patients taking an anticoagulant. This post requires that you have the ability to travel between locations to carry out your work and to attend training and visits; daily access to transport is therefore essential. The ability to commute within CHCP CIC's geographical areas with access to appropriate means of transport is essential - a full UK driver's license is required.
