Advanced Nurse/Clinical Practitioner
The ANP/ACP plays a critical role in supporting virtual care by providing support and advice to our remote monitoring clinicians.
The post holder will work as an experienced, autonomous Practitioner who, acting within their professional boundaries will provide care for patients remotely and from history taking, clinical assessment, diagnosis, treatment and evaluation of care, you must demonstrate critical thinking in the clinical decision-making process to ensure safe and expert care.
The post holder will be able to demonstrate safe, competent clinical decision making and deliver expert care, including assessment, diagnostic skills, treatment and evaluation. They will communicate at all levels across boundaries and work closely with wider clinical team members. The post holder will also work collaboratively with the multidisciplinary team to meet the needs of patients, supporting the delivery of policy and procedures, and providing Clinical leadership as required.
The post holder will use the web-based Doccla platform to help monitor patient wellbeing, support their care needs, and assist in delivering high-quality care in collaboration with the wider clinical team.
Main duties of the job
Autonomous Clinical Care: Provide high-standard, independent diagnosis and treatment for acute and complex conditions.
Remote Consultations: Conduct patient assessments via telephone and video, supporting remote monitoring teams with vital sign data.
Advanced Diagnostics: Request and interpret invasive/non-invasive tests and refer patients to specialists or secondary care when needed.
Independent Prescribing: Safely prescribe medications (V300) and advise patients on drug regimens and side effects.
Long-Term Condition (LTC) Management: Identify and manage treatment plans for chronic illnesses, empowering patients to self-manage at home.
Emergency Intervention: Prioritize urgent health problems and initiate emergency care or escalations when necessary.
Clinical Governance: Maintain accurate electronic patient records, participate in clinical audits, and support quality improvement.
Patient Wellbeing: Offer remote reassurance and advice, ensuring patient safety and comfort within their home environment.

- Familiarity with digital healthcare platforms.
