Job summary
We are looking for a motivated and compassionate PCN Clinical Pharmacist to join GP surgery sites in N22 and N8, Haringey, North London. This is an excellent opportunity to contribute to high-quality patient care within a supportive general practice environment. You'll work closely with GPs, pharmacists, other healthcare professionals and administrative staff to improve medicines optimisation, support long-term condition management, and enhance patient outcomes.

You'll support patients with complex medication needs, contribute to the management of long-term conditions, and help optimise prescribing practices. The role also involves collaborative working across teams and organisations to promote consistent, high-quality care.

- Carrying out medication reviews to support patients with complex needs, polypharmacy, long-term conditions, and acute medication queries.
- Supporting the practices' medicines optimisation, repeat prescribing systems, and medication safety processes.
- Providing evidence-based prescribing advice to clinical teams.
